Policies

NOTICE TO ALL PATIENTS—PLEASE READ

Please Present Your Insurance Card Upon Every Visit

Due to constantly changing insurance companies, we ask that you present your insurance

card upon every office visit. This will prevent any cause for delay in processing your insurance claim.

Co-Payment

Your insurance now requires us to ask and collect your

insurance co-pay before services are rendered.

Refills

Please make sure that you ask for and receive all refills on any medication that you may

need to last until your next doctors appointment during today’s visit. We will no longer call in

any refills on medication. If refills are needed an office visit will be required.

Medications

For your safety, you must bring all medications to each visit. or your appointment may be rescheduled.

Prior Authorizations

All prior authorizations on medication will need to be

approved as medically necessary by Dave Ringer, M.D., FAAFP.

No-Show Policy

We ask that you notify us at least 24 hours prior for any appointment cancellation,

or as soon as possible. Patients that do not show for their appointment will be

charged a $25.00 self-pay fee. We will not be able to reschedule your appointment

until the No-Show charge is paid.

Medical Records

Medical Records are performed by Discovery Health Services, a private Medical Record Company.
