Bob Perry, is married
to Jean H. Perry. He has a son and one daughter and lives in Sparta
Georgia.
Military History
From 1968-1969 he attended Georgia Southwestern College, in Americus, Georgia.
In 1969 he enlisted in the US Army and completed basic training at Fort Knox,
KY. He then went onto complete advanced individual training at Fort Sam in Houston,
TX - Medical Corps. While in services he attended the 91C20 Advanced Training
Program at Womack Army Hospital in Fort Bragg, NC. Also, during 1971-1973 he
was a Clinical Technician in charge of the 9th Medical Dispensary, in Aschaffenburg,
Germany. Bob Perry was honorably discharged from the US Army - Rank - Specialist
5 on January 3, 1973.

Education, Employment History and Accomplishments
In 1976 he graduated from the Physician Associate Program at Emory University
School of Medicine in Atlanta, GA. He began Extern Rotations in family
practice at the Medical Center of Central Georgia in Macon, GA. He passed
the National Commission on Certifications of Physicians Assistants Board
Exam #770945 in 1976.
From 1977-1984 he was employed by Dr. George F. Green, Sparta Medical
Clinic, Sparta GA. During that time he was granted a State License by
Composite State Board of Medical Examiners #000234; Granted Staff Privileges
at Putnam General Hospital, Eatonton, GA; became a Member of American
Academy of Physician Assistants, #0006027. In 1979 he served as Membership
Chairman
GAPA and in 1981 re certified by NCCPA-#770945.
In 1984 - 1986 he was employed by Dr. M. Lynn Ferrell, at Sparta Medical
Clinic, Sparta, GA. In 1986-1995 he was employed by Dr. Rakesh Kumar,
Eatonton, GA. During this time, in 1989, he was elected to the Board
of Trustees, John Hancock Academy, in Sparta, GA and served on the board
through 1994.
In 1995 he was re certified by NCCPA #770945. In 1996 he began employment
at Putnam Family Medicine in Eatonton, GA for Centra Management Company.
He was re certified by NCCPA #1003585 in March of 2001 and in March of
2004 he began at Family Medical Associates in the Lake Oconee. In 2005 he was elected Georgia's Physician Assistant of the Year. |
